Health Screening and VaccinationHealth Screening, Vaccinations and Declaration RecordDescriptionHealth Declaration Screening (please select the products you require) At Ulster University one of our primary concerns is to ensure that you are safe during your time here as a student and medically fit for your placements. As a condition of your offer of study and part of the pre-registration processes, you have been asked to complete a Health Declaration Form which will include information about your vaccination history. The University Occupational Health Nurses will screen your form. Following screening of your form, it may be necessary for you to meet with a nurse or for a medical to be arranged with the University Occupational Health Physician. We therefore require you to return your completed form as soon as possible. Failure to do so may result in a delay in your registration on the course as you will not be permitted to register until you have been passed ‘medically fit’.
Vaccination and Immunity Programme Healthcare workers have a duty of care towards their patients which includes taking reasonable precautions to protect them from communicable diseases. Any vaccine-preventable disease that is transmissible from person to person poses a risk to both healthcare professionals and their patients. You will need to consult your GP before completing the Health Declaration Form to obtain your vaccination record. You will need to attach this record to your Declaration of Health form.

You will need to make a payment for the health screening and any vaccinations you require prior to submitting your health declaration form. Please ensure you print the email confirming your payment and attach to the Declaration of Health form. Should you require any vaccinations, the date will be confirmed as soon as possible after your studies start. Please note you will be required to bring photographic ID when you attend for vaccinations and serology tests. |
